Run title, abstract, and full-text screening with reviewer control
This guide explains how to configure an AccuScreener project, choose Manual, AI Assistant, or AI Screener workflows, import studies, manage duplicates, run first-pass and full-text screening, prepare PDFs, resolve conflicts, and export auditable decisions.
What AccuScreener does
AccuScreener is the SYMPRO AI workspace for title-and-abstract screening and full-text review. It helps teams configure eligibility criteria, assign human screeners and reviewers, decide how AI participates, compare decisions, adjudicate conflicts, and retain a decision trail for export.
The tool is built around researcher control. AI can assist a human screener or act as an independent screener, but eligibility decisions, conflict resolution, protocol choices, and downstream interpretation remain the responsibility of the review team.
| What you provide | What AccuScreener manages | What you review or export |
|---|---|---|
| Project details, protocol, publication types, and eligibility categories | Versioned screening configuration and role-based work queues | Protocol versions, active categories, and readiness status |
| Studies from AccuSearch, Excel, RIS, or NBIB | Study records, duplicate detection, assignments, and progress tracking | Screening decisions, duplicate groups, conflicts, and reviewer actions |
| Full-text PDFs and FTR configuration | PDF matching, extraction status, AI runs, and FTR readiness checks | Full-text decisions, Mini-DET data, exports, PRISMA data, and audit timeline |
The complete AccuScreener workflow
Screening moves from project setup to first-pass review, then to PDF preparation and full-text review. Later stages depend on the configuration and decisions made earlier, so changes should be reviewed before relying on exports or downstream handoffs.
- 1
Create or link the project
Start a standalone screening project or open the screening stage inside a master project workflow.
- 2
Configure protocol, criteria, and team
Set project details, select screening type, enable AI mode if needed, assign analysts, select the eligible reviewer pool, and confirm eligibility categories.
- 3
Add studies and resolve duplicates
Load records from AccuSearch or upload Excel, RIS, or NBIB files, then apply automatic or manual duplicate review.
- 4
Complete first-pass screening
Screen titles and abstracts manually, with AI suggestions, or against an independent AI Screener decision stream.
- 5
Prepare full-text review
Upload or auto-download PDFs, match files to studies, resolve text extraction status, publish FTR configuration, and clear readiness blockers.
- 6
Review, adjudicate, and export
Complete FTR, resolve required FPS and FTR adjudication, inspect audit history, and export screening decisions, PRISMA data, and timeline records.
Choose how AI participates before screening begins
AccuScreener supports manual screening, visible AI assistance, and independent AI screening. The mode changes what screeners can see, how conflicts are created, and how reviewers adjudicate final eligibility.
| Mode | How it works | User responsibility |
|---|---|---|
| Manual | Human analysts screen records without AI participation. | Definitive single review decisions or compatible double review agreement can finalize without review; Unsure and conflicts require adjudication. |
| AI Assistant | AI recommendations are visible to assigned users as screening suggestions during FPS and supported FTR work. | Users make the screening decisions. The AI suggestion is not a separate arm and does not replace human adjudication. |
| Double Review (AI+Humans) | AI owns Arm A while human analysts independently screen assigned Arm B records. | Compatible AI-human agreement can finalize without review; Unsure or conflicts create independent human reviewer work. |
| AI Screener - sole screener | AI is the only screening stream and human analysts do not submit screening-arm decisions. | Definitive AI Include or Exclude results become final immediately. Reviewers resolve AI Unsure results and may optionally sample definitive AI decisions. |
Assign analysts, reviewers, and project-level responsibility
Project setup separates screening work from adjudication work. Analysts screen assigned records, while the selected reviewers form an eligibility pool. Reviewer work is not distributed to every study during setup; it is created later only when a completed FPS or FTR outcome requires adjudication.
| Role or setting | Purpose | Watch for |
|---|---|---|
| Administrators | Create projects, configure criteria, manage studies, run duplicate detection, prepare AI runs, and monitor progress. | Configuration and source-set changes can affect downstream queues and exports. |
| Analysts | Screen assigned studies during first-pass or full-text review when the selected mode allows human screening. | Unassigned studies, locked projects, and AI-only modes can prevent decision submission. |
| Reviewers | Resolve Unsure decisions and screening conflicts, and optionally review definitive decisions in sole AI Screener projects. | The project selection is an eligibility pool. Required work appears only when a study needs resolution; sole-AI reviewers can also open the optional AI review queue. |
| Single review | Supports one main screening decision stream with reviewer oversight where configured. | Use when the protocol does not require independent dual screening. |
| Double review | Supports independent decision streams before comparison and adjudication. | Use when the protocol requires separated human or AI-human screening decisions. |
Configure the screening rules that drive decisions
AccuScreener uses protocol data, publication types, and inclusion or exclusion categories to guide first pass screening, full-text review, AI runs, reviewer queues, and exports. These settings should match the approved review protocol before screening starts.
| Configuration area | What it controls | Review guidance |
|---|---|---|
| Protocol data | Population, intervention, comparator, outcomes, study design, country, and related eligibility context. | Keep enabled fields aligned with the approved protocol and active screening questions. |
| Decision categories | Inclusion and exclusion criteria used when users choose Include, Exclude, or Unsure. | Use categories specific enough to explain decisions without creating unnecessary overlap. |
| Publication types | Article types that are eligible for the project. | Blank or excluded publication types can remove records from eligibility, so confirm imported values. |
| FTR configuration | Full-text review questions, Mini-DET fields, required data extraction, and linked protocol version. | Publish a default FTR configuration before enabling full-text review. |
| Version history | Draft, published, default, and archived protocol or FTR configurations. | Use versions to document which rules governed each screening or AI run. |
Add studies from AccuSearch or supported uploads
A screening project can receive records from a linked AccuSearch workflow or from uploaded study files. Confirm the source set before screening so analysts and AI process the intended records.
| Source | Use it when | Important context |
|---|---|---|
| AccuSearch import or sync | The search strategy and article retrieval were prepared in AccuSearch. | Syncing adds or updates records but does not automatically run AI screening. |
| Excel upload | The team already has a structured citation workbook. | Required columns and positive whole-number IDs keep screening decisions traceable. |
| RIS upload | Records come from citation managers or databases that export RIS. | Project IDs can be assigned while source identifiers remain available for reference. |
| NBIB upload | Records come from PubMed or PubMed-compatible exports. | PMID-style source identifiers can be retained while AccuScreener manages project IDs. |
Excel upload expectations
- Required columns include ID, Author, Year, Title, Journal, Abstract, DOI, and Article type.
- Study IDs should be positive whole numbers so decisions and exports remain traceable.
- RIS and NBIB imports can assign project IDs while retaining source identifiers.
- Upload previews should be checked before adding records to the project.
Resolve duplicate records before they distort screening
Duplicate detection keeps multiple citations for the same study from creating redundant work or conflicting decisions. AccuScreener supports automatic duplicate marking and manual duplicate review.
| Duplicate mode | How records are matched | Best used when |
|---|---|---|
| Auto | Applies the fixed duplicate rules immediately, including DOI, Title plus DOI, or Title plus Year when DOI is missing. | Projects that want duplicate groups applied quickly with standard criteria. |
| Manual | Lets administrators combine fields such as DOI, Title, Authors, Year, Volume, Issue, Journal, and Pages with AND logic. | Projects that need reviewable duplicate rules before records are marked. |
Complete first-pass screening with clear decision context
First-pass screening uses study titles, abstracts, publication information, protocol data, and configured categories to capture Include, Exclude, or Unsure decisions. The visible information changes by role, assignment, project mode, and whether AI Assistant or AI Screener is active.
| Decision context | What users see | Why it matters |
|---|---|---|
| AI Assistant visible suggestion | The AI decision, reason, comment, and criteria-level results can be shown to the user. | The human screener can use AI as context while still making the decision. |
| AI Screener hidden decision | Human screeners do not see the AI decision during independent screening. | The system preserves blinded comparison between AI and human decisions. |
| Duplicate warning | A warning appears when the current study is part of a duplicate group. | Screeners should avoid treating duplicate records as independent evidence. |
| Conflict banner | Decision conflicts are surfaced when decisions disagree. | Reviewers need a clear queue for adjudication. |
| Advanced screening | Search, sorting, pending/done tabs, reviewer filters, and bulk actions are available where permitted. | Large projects need efficient worklists without losing decision traceability. |
Use Advanced Screening for controlled bulk decisions
Advanced Screening is an assignment-scoped FPS worklist for finding studies and applying the same decision to multiple selected records. It does not provide access to studies that are not assigned to the current screener or reviewer.
| Control | How to use it | Safety rule |
|---|---|---|
| Work arm | In double review FPS, choose All assigned, Arm A, or Arm B before selecting studies. | Only the current user’s assignments in that arm are loaded; the other screener’s decision remains hidden. |
| Search and filters | Use simple search or combine metadata filters with AND/OR, then sort and page through the results. | Changing a tab, arm, search, filter, or page clears the current selection. |
| Bulk decision | Select the visible studies and choose Include, Exclude, or Unsure. | Include and Exclude require an active criterion when the protocol defines categories. |
| Protocol version | Check the protocol version shown in the decision dialog before applying the batch. | The server validates the criterion against the active protocol again when each decision is saved. |
| Partial result | Review the result banner after Apply. | Successful studies remain saved; every failed study stays unchanged and is listed with its reason. |
Use AI runs as auditable screening operations
AI Runs create traceable batches for first-pass screening or full-text review. Each run is tied to the selected protocol and, for FTR, the selected FTR configuration, so the decision context can be reviewed later.
| AI run rule | Meaning | User action |
|---|---|---|
| Preview before run | Previewing an AI run does not start processing or charge credits. | Check scope, estimated workload, and configuration before starting. |
| New screening | Targets eligible studies that do not already have an AI decision for the selected stage. | Use when AI screening has not yet been run for those records. |
| Re-screening | Runs AI again and can replace current AI results while preserving history. | Use after meaningful protocol, source, or review-scope changes. |
| Version snapshot | FPS runs use a protocol version; FTR runs use an FTR configuration and linked protocol. | Confirm the intended default or selected version before processing. |
| AI Screener recalculation | Replacing AI Screener results can update the AI analyst entry and final decision calculations. | Review conflicts and adjudication queues after re-screening. |
Prepare PDFs before enabling full-text review
Full-text review depends on eligible first-pass decisions, matched PDFs or explicit PDF skips, text extraction, and a published FTR configuration. The PDF Manager helps teams upload, match, retry, skip, and audit files before FTR begins.
| Readiness area | What to check | Effect on FTR |
|---|---|---|
| PDF matching | Primary PDF filenames should match the study ID pattern, with supported supplementary naming for appendices or datasets. | Matched files become available in the FTR viewer and AI processing pipeline. |
| Auto-download | Open-access DOI records can be checked for automatic PDF retrieval where supported. | Eligible records may receive PDFs without manual upload. |
| Text extraction | Extraction statuses such as extracted, pending, failed, unavailable, no PDF, or skipped need review. | Full-text AI requires usable extracted text. |
| Skipped PDFs | Studies can be explicitly skipped when a PDF is unavailable or not needed for AI processing. | Skipped records can proceed without blocking FTR setup for other studies. |
| FTR readiness | Default FTR configuration, unresolved FPS decisions, PDF state, extraction state, and eligibility reviews are checked together. | Unsure and decision conflicts block FTR; agreed Include with a criteria-only conflict may proceed while its FPS review remains open. |
Use FTR configuration, PDFs, and reviewer decisions together
Full-text review combines the study list, PDF viewer, review questions, Mini-DET fields, and decision dialogs in one workspace. FTR evaluates its reviewer requirement separately from FPS. Decisions are associated with protocol and FTR configuration versions so later exports can show the rules used for the work.
| FTR element | Purpose | Review guidance |
|---|---|---|
| PDF viewer | Shows primary or supplementary PDFs and supports review inside the FTR workspace. | Confirm the file belongs to the study before relying on full-text decisions. |
| Review questions | Captures configured full-text eligibility prompts. | Keep questions consistent with the published FTR configuration. |
| Mini-DET fields | Collects configured data extraction values during FTR. | Review auto-save status and missing values before final export. |
| AI Assistant FTR | Displays AI guidance when available in AI Assistant mode. | Treat missing or error states as AI-run issues for administrators to review. |
| Version labels | Shows the protocol and FTR configuration versions tied to decisions. | Use labels to understand whether older decisions used previous rules. |
Reviewer work is assigned when a study needs it
Selecting reviewers during project creation defines who is eligible to adjudicate; it does not assign every study to a reviewer. After the required analyst or AI screening decisions are saved, AccuScreener evaluates that study and either finalizes an accepted outcome or creates one reviewer task.
| Screening mode | Screening result | Reviewer assignment | FTR |
|---|---|---|---|
| Single review (human) | Include | None required; Include becomes final | Eligible |
| Single review (human) | Exclude | None required; Exclude becomes final | Not sent |
| Single review (human) | Unsure | Assign an independent reviewer | Wait for reviewer decision |
| Double review (two humans or AI + human) | Both arms Include, with compatible criteria | None required; Include becomes final | Eligible |
| Double review (two humans or AI + human) | Both arms Exclude, with compatible criteria | None required; Exclude becomes final | Not sent |
| Double review (two humans or AI + human) | Unsure outcome or decision conflict | Assign an independent reviewer; see When adjudication is required below | Wait for reviewer decision |
| Double review (two humans or AI + human) | Both arms Include, but criteria differ | Assign an independent reviewer to resolve the criteria; review may remain pending | Eligible while criteria review is pending |
| Double review (two humans or AI + human) | Both arms Exclude, but criteria differ | Assign an independent reviewer to resolve the criteria | Not sent |
| Any mode | A required screening decision is still Pending | No reviewer task yet | Wait for screening |
| Any mode | Duplicate secondary record | No ordinary reviewer task | Not sent through ordinary FTR workflow |
AI Assistant provides advice, not an independent screening arm. Follow the applicable human single or double review rule. Sole AI Screener follows the separate rules below.
How AccuScreener selects the reviewer
- Wait until all required screening arms for the phase have submitted their decisions.
- Classify the outcome as no adjudication required or review required.
- Start with the active reviewers selected for the project and remove anyone who screened that study.
- Assign the eligible independent reviewer with the lowest unresolved workload. Ties use a stable rotation so the result is predictable.
- If nobody is eligible, save the screening work and mark the study Review required, unassigned for administrator action.
| Reviewer status | What it means | What happens next |
|---|---|---|
| Awaiting screening | The FPS or FTR decisions needed to evaluate reviewer eligibility are not complete. | Complete the required analyst or AI screening arms. |
| No adjudication required | A definitive single review outcome or compatible double review agreement has been accepted automatically. | The final decision is available; no reviewer is assigned. |
| Review required, unassigned | The outcome needs adjudication, but no active independent reviewer is currently eligible. | An administrator must add or reassign a suitable reviewer without resetting completed analyst work. |
| Review pending | An active independent reviewer has been assigned and has not submitted the adjudication. | The study appears only in that reviewer’s applicable FPS or FTR queue. |
| Resolved | The assigned reviewer submitted the final adjudication. | The adjudicated decision is final and retained in history and exports. |
Examples
| Example | Result | Reviewer handling |
|---|---|---|
| Single review: Sanjana selects Exclude | A definitive single-stream decision is available. | Finalizes as Exclude with no adjudication required. |
| Single review: Sanjana selects Unsure | The outcome cannot be finalized from the analyst decision. | One active reviewer independent from Sanjana is assigned. |
| Human double review: both arms select Include | The definitive decisions agree and their criteria are compatible. | Finalizes as Include; no reviewer task is distributed. |
| Human double review: both arms select Exclude but choose conflicting criteria | The decision agrees but its eligibility rationale conflicts. | An independent reviewer is assigned to resolve the criteria. |
| AI+Human: AI selects Exclude and Amit selects Include | Arm A and Arm B have a decision conflict. | A reviewer other than Amit is assigned. |
| Sole AI: AI selects Include | The definitive AI decision becomes final immediately. | The study can proceed toward FTR; a selected reviewer may optionally verify or override it. |
FPS and FTR are separate reviewer lifecycles
An FPS reviewer assignment is not copied into FTR. Unresolved FPS Unsure and decision conflicts block ordinary FTR. If both FPS arms agree on Include but their criteria differ, the study can enter FTR while its FPS criteria review remains pending. When FTR screening is later completed, AccuScreener evaluates the FTR decisions independently and creates FTR reviewer work only if the FTR outcome requires it.
Reviewer statistics and worklists count actual review work. Administration statistics separately identify outcomes finalized without adjudication and studies that require review but do not yet have an eligible reviewer. Duplicate secondary records are handled through the duplicate workflow and are not distributed as ordinary reviewer tasks.

Assign master reviewers for FPS, FTR, or both during team setup or in Change Team. Master review becomes available after screening is complete for the sole screener or one full arm in a double-blind project. Master decisions take precedence for their phase and retain an audit history.
The dedicated walkthrough explains All studies and non-overlapping random sets, Reading and Excel-style views, filters, bulk decisions, and what happens to FTR when a master reviewer excludes FPS.
verified_userOpen the Master Reviewer guideDefinitive AI decisions can finalize while human review remains available
Sole AI Screener is a single review workflow. A definitive AI Include or Exclude becomes the final FPS decision immediately, so an included study can proceed toward FTR without waiting for every record to be reviewed. AI Unsure and failed or unfinished AI processing remain blocked. Selected project reviewers can optionally inspect definitive AI decisions; when a reviewer submits a decision, that human decision becomes final and can verify or override the AI result.
FTR has its own Optional AI review queue for definitive, unreviewed AI FTR Include or Exclude results on FTR-eligible studies. Selected independent reviewers can verify or override those results without waiting for a required assignment. The ordinary FTR All assigned reviews list continues to show only the reviewer's assigned or completed work; pending AI results, Unsure outcomes, duplicates, and studies requiring adjudication do not enter the optional queue.
| AI result | Final FPS decision | Reviewer handling | FTR |
|---|---|---|---|
| Pending/failed | Pending | None | Blocked |
| Include | Include from AI | Optional review | Eligible immediately |
| Exclude | Exclude from AI | Optional review | Not sent to FTR |
| Unsure | Pending/Unsure awaiting resolution | Assign a human reviewer | Blocked |
| Duplicate | System Exclude | No reviewer | Not sent to FTR |
AI Reviewer FPS Statistics
Sole AI Reviewer replaces the ordinary FPS reviewer table with a sole-AI-specific table. It separates required resolution work from optional quality-assurance samples and shows whether completed human reviews agreed with or changed the AI result.
| Column | Definition |
|---|---|
| Reviewer | The human reviewer who received required resolution work or completed an optional AI quality-assurance review. |
| Required | Studies assigned because the AI result could not finalize, such as AI Unsure. |
| QA Sample | Definitive AI Include or Exclude studies voluntarily reviewed for quality assurance. |
| Pending | Assigned required reviewer tasks that have not yet received a human decision. |
| Completed | Required and optional reviews for which the reviewer submitted a decision. |
| Verified AI | Completed human reviews where the normalized reviewer decision matches the AI decision. |
| Overrode AI | Completed human reviews where the reviewer decision differs from the AI decision; the reviewer decision becomes final. |
| Total Assigned | Required plus QA Sample reviews associated with that reviewer. |
| Completion | Completed reviews divided by Total Assigned. |
When adjudication is required
Once the required screening decisions are complete, Unsure outcomes and decision or criteria conflicts create reviewer work. The same classifications apply whether Arm A is screened by AI or a person.
| Example | Classification | Where the assigned reviewer sees it | FTR before review |
|---|---|---|---|
| Arm A Unsure; Arm B Include | Unsure outcome — one arm could not make a definitive decision | All assigned reviews and Unsure to resolve | Blocked |
| Arm A Include; Arm B Exclude | Decision conflict — two definitive decisions disagree | All assigned reviews and Decision conflict | Blocked |
| Arm A Include (I1); Arm B Include (I2) | Criteria conflict — decisions agree, but selected criteria differ | All assigned reviews and Criteria conflict | Eligible while criteria review is pending |
| Single review analyst selects Unsure | Unsure outcome — a definitive decision is needed | All assigned reviews and Unsure to resolve | Blocked |
The reviewer dropdown groups unfinished work by its reason: Unsure to resolve, Decision conflict, or Criteria conflict. All assigned reviews includes unfinished and completed work, with unfinished studies first; it also retains any legacy task without a saved reason. FPS additionally has My Unsure decisions for Unsure outcomes submitted by the reviewer. FTR reviewers must choose Include or Exclude; an unresolved study should be left open, not finalized as Unsure. If no independent reviewer is eligible, the study is marked Review required, unassigned instead of entering anyone's worklist.
Know when earlier work needs review
Screening is cumulative. Changes to the source set, protocol, FTR configuration, team assignment, AI runs, or first-pass decisions can affect downstream queues and outputs.
| Change | Possible effect | Recommended review |
|---|---|---|
| Protocol or category update | New work may use different eligibility context than earlier decisions. | Publish the intended version and check whether AI re-screening is needed. |
| FTR configuration update | Review questions or Mini-DET fields may differ from previous full-text work. | Publish the default FTR configuration and confirm readiness again. |
| AccuSearch sync or new upload | The source set can gain new records or match existing records. | Run duplicate checks and decide whether AI or human screening should cover the additions. |
| AI re-screening | Current AI results can be replaced while historical results remain auditable. | Inspect refreshed conflicts, reviewer queues, and final-decision calculations. |
| Team reassignment | Workloads and pending queues can change while screening is in progress. | Use locking during reassignment when needed to avoid decision changes mid-distribution. |
| FPS decision changed after FTR | Existing FTR work may no longer match first-pass eligibility. | Complete the eligibility review before considering FTR outputs final. |
Use outputs with their decision context
AccuScreener exports and timeline views are designed to show not only the final decision, but also the setup, reviewers, AI runs, duplicate handling, PDF status, and history behind that decision.
| Output | What it contains | Use it for |
|---|---|---|
| Screening export | Study metadata, FPS decisions, reviewer decisions, duplicate audit data, and final screening outcomes. | Document title-and-abstract eligibility and adjudication. |
| Full-text export | FTR analyst, reviewer, final decisions, criteria, review questions, and Mini-DET or extraction fields. | Prepare full-text eligibility and extraction records. |
| PRISMA data | Screening counts and flow information derived from the project decision state. | Support reporting of the study selection process. |
| AI run history | Run scope, processing stage, selected versions, results, and re-screening context. | Audit where AI contributed to screening. |
| Screening timeline | Setup, imports, duplicates, PDFs, AI activity, lifecycle events, and article decision events. | Investigate who changed what, when, and under which configuration. |
How PRISMA chooses inclusion and exclusion reasons
PRISMA uses the saved final decision to count included and excluded studies. For each study, it selects one reason for the reported stage and outcome: FPS exclusion, FTR exclusion, or FTR inclusion. An eligible reason must be non-empty and belong to a completed decision matching that final outcome.
| Project type | Reason priority, highest first |
|---|---|
| Single-blind | Active master reviewer → Reviewer → Analyst |
| Double-blind: AI in Arm A, human in Arm B | Active master reviewer → Reviewer → Arm B (human) → Arm A (AI) |
| Double-blind: both arms human | Active master reviewer → Reviewer → Arm B → Arm A |
If both arms exclude with different reasons and no eligible reviewer reason is available, Arm B's reason is used. The same priority applies to FTR inclusion reasons. If Arm B has no eligible reason, Arm A is considered. For an agreed Include with an FPS criteria conflict, a pending reviewer does not supply a reason, so Arm B's matching reason is preferred where that FPS reason is reported. A completed reviewer reason takes priority later. The order in which analysts submit decisions does not determine arm priority.
- Pending or unresolved final decisions do not contribute inclusion or exclusion reasons.
- If a final outcome has no eligible reason, the study is counted under “Reason not recorded”.
- Each study contributes only once to its stage's reason breakdown; reasons from both arms are not added together.
- FPS inclusion totals also include Unsure; FTR inclusion totals require an Include outcome.
